If you are using the original patch, it has some issues with lame and
failing servers. See:

solution, though, is moving the duplicate suppression lower, as in:

I haven't tried it with the ipv6 patch, though; there are likely to be
some conflicts, as the remote IP address is used as part of the key for
determining duplicates.
