Confirm that '10.11.12.1' is really the IP of the computer running the
tftp server. Check that there is a pxelinux.0 in the tftp directory on
that computer. Check that the tftp server is running(i.e. 'ps aux
|grep tftp').
If you confirmed all that, try to connect to the tftp server using a
tftp client on another PC running Windows or Linux(I think windows has
one out of the box, there's 'tftp' for linux).

Please also paste your tftp server config and the relevant parts of
the DHCP server config file.
