I'm definitely not 100% set on using XEP-0224, I just considered it a good potential reuse of our existing tech, but if you believe it is not, I'm happy to be convinced otherwise. That said, XEP-0224 is clearly not meant to be used in the way you described. It should not affect the display of the message itself, it should only draw the users attention to the chat session. The <attention/> tag can even appear without a body. The implementation notes list various ways to implement it, but none of them affect the display of the message itself after the attention was grabbed.
