These types of questions are best addressed to [email protected].  

Fiber cables are quite resistant to damage.  If the green LED is
off, the cable is dead.  There are two possibilities when a cable
doesn't work anymore.  Either the tip of the fibers are covered
with dust (that's why there are white caps on end of the fibers
on the connector when it's not in use).   The more common
problem is that the fiber thread is broken inside the cable.
That could happen if the fiber is bent at a very sharp angle.
Loops are okay.

This cable was purchased through a Myricom reseller or
was it purchased directly from Myricom?  An RMA needs
to be issued for this cable.  If you purchased through a
reseller, then you need to contact them about RMA'ing
the cable.  If you purchased directly from Myricom, then
we will issue the RMA and give you instructions for
returning the cable for replacement.  When was the 
cable purchased?
