I just wanted to follow this up with our solution. It turns out that we had terrible and sometimes subtle problems due to some inadequate PCI extender cables. We found two solutions that were successful. One was to use impedance-matched Kapton cables with a ground plane. The second, the one we used, was to use some custom riser cards from Adex Electronics.
