The main "back-end" system (for actually gathering and storing the data) is RRDTool - see http://people.ee.ethz.ch/~oetiker/webtools/rrdtool/ There are several front-ends to this - including MRTG (which was the forerunner of RRDTool), Cricket, and Cacti (which seems to be one of the most popular now).
