How does this information get passed between the process and the
agent?  Does the agent contact the process when it receives a
request, or does the process know about the SNMP agent (and inform
it when it start up)?
  That will affect the answer to your first question:

Any of the mib2c templates could be appropriate, depending on
the information you're working with, how the agent and process
communicate, etc.

A little more information about your setup would be useful.
