If the table indexes are defined as IMPLIED OCTET STRING
(or IMPLIED OBJECT IDENTIFIER), then the encoding won't
include the length subidentifier.

Similarly, if they are defined as fixed-length OCTET STRINGs

If the indexes are defined as non-IMPLIED (and variable
length), then they will always include a subidentifier to
indicate the correct length.

See RFC 1902 for when table indexes can and can't use
IMPLIED.
