tools to monitor per-process CPU/memory?

When I snmpwalk a Linux or Windows host I can see values returned for a
list of proceses that might include:
HOST-RESOURCES-MIB::hrSWRunName.1350 = STRING: "httpd"
and an assortment of associated things like:
HOST-RESOURCES-MIB::hrSWRunPerfCPU.1350 = INTEGER: 1885
HOST-RESOURCES-MIB::hrSWRunPerfMem.1350 = INTEGER: 400 KBytes
etc., but of course the indexes are likely to change frequently.
Are there any higher level tools that can monitor per-process
CPU/memory, etc., given a process name?
