The simplest approach is probably to write a little shell script
that does something like:

    fired=0
    n=3      #  the index of the Db row you wish to monitor.
    while true
    do
         size=`snmpget -Ov ....  mssqkIDbInfoSize.$n`
         if [ $size -gt 6000 ]
         then
              if [ ! $fired ]
              then
                  snmptrap .....
                  fired=1
              fi
         else
              fired=0
         fi
     done


There is a way to handle this from within the agent itself
(see the section on "Active Monitoring" in snmpd.conf(5)),
but there are currently problems with monitoring data
from subagents.
   I suspect the script above (suitably adapted for your needs)
may be the simplest in the short term
