Does it work OK if you omit your MIB module?
  (i.e. confirm that it is definitely your code that is triggering the problem)

Assuming that this problem *is* in your module, it might be
helpful if you posted the initialisation code in question.
   How can we possibly guess what is wrong with code
that we've never seen?!
