Err....
   why have you got *two* extensions, both providing the
same information, and rooted at the same OID?
    That's completely unnecessary. confusing, and I'm
surprised it even works.

Just use *one*, and delete the other.
Preferably keep the "extend" directive, since the relocatable "exec"
format is being withdrawn.
