If there's a MIB object that indicates the health of a subsystem, then you could use the DisMan Monitoring MIB to keep a watch on this object, and issue a trap when the value changes (or takes on a given value). See the section ACTIVE MONITORING in the snmpd.conf(5) man page for more details.
