These may be obvious questions, but:
   - does the Cisco box support SNMP access over IPv6?
   - if so, is this support enabled?    (either by default, or explicitly)
   - does the box limit who can query it?
   - if so, is the system you are using as a management station,
     configured as an authorised source?

It might be worth checking SNMP-over-IPv4, just to see if that's
working or not.
