I use smsclient (www.smsclient.org) and these are my commands:

command[notify-by-epager]=/usr/bin/sms_client -q $CONTACTPAGER$ "$NOTIFICATIONTYPE$:
$HOSTADDRESS$ $HOSTALIAS$: $SERVICEDESC$ is $SERVICESTATE$"

command[host-notify-by-epager]=/usr/bin/sms_client -q $CONTACTPAGER$
"Host $HOSTALIAS$ is $HOSTSTATE$; $OUTPUT$; $DATETIME$"
