I'm not sure what this really means. slrn doesn't normally show groups with 
0 unread articles in them after fetching new news (you can make slrn do 
this if you want slrn to behave otherwise). So refreshing the newsgroup 
list should only show groups with unread articles in them and that article 
count would naturally include articles which arrived on your NNTP server 
after you left the group (including your posts to that group).

The only way I could make sense of your request and come up with something 
implementable in a macro (meaning no slrn code changes) would be to use a 
hook to move a group you just left to the top of the group list if you quit 
article mode with at least 1 unread article in the group, otherwise move 
the group to the bottom.

Would that suffice?

As far as I recall, group mode doesn't allow modification of the group name 
displayed (of course slrn is free software, so you're free to modify slrn's 
code to do what you want).
