I think the first pulldown list is asking which device do you want to forward to. It should list all active devices on your network. Pick the one running your TightVNC server. The second pulldown refers to the application you want to forward. In this case it is VNC. If there is no VNC option then set it up manually and open port 5900 using both TCP and UDP. If you have multiple machines running a TightVNC server and you want to be able to connect to any one of them, configure each server to use a different port (e.g. 5901, 5902, etc.) and set up a different port forwarder for each, setting the port on the router to match. If the device you want to access is a Linux machine, it's more secure to forward port 22 (ssh) to it then use local port forwarding on ssh to create a tunnel but this is a different topic.
