What version of xinetd are you running? You should try to run the
latest RH release as it fixes most of the known bugs.

Also when you do a "service xinetd start", what do you get in
/var/log/messages?

I'm not 100% sure qmail runs from xinetd. pop3/imap might, but
the actual smtp would be unusual.
