Have you considered PenTeleData?  I used to be a customer of them years 
ago, and although they had the usual 'gatekeeper' mentality between 
their sales reps and their technicians ("We can't tell you what the T1 
framing and coding are, because you wouldn't buy your CSU/DSU from us!" 
followed by "We can't tell you what your /25 assignment is, because you 
wouldn't buy your Cisco 2514 from us at inflated prices!"), they seemed 
to have a decent network and kept with the times.
