When I worked at Hawaiian Telcom there was a one
time charge for the setup, but there was no MRC.
Seemed a reasonable charge as there aren't many 
folks here that do BGP and they constantly do 
things like advertise the entire internet to us.
Sorting these and other things out for the new
customers took a good bit of time for most BGP
sessions.
