I did this about eight years ago at http://coza.posix.co.za One can fill in
the form - etc, including adding your own Nameservers, which are cross
checked to satisfy the COZA requirements.

Totally agree. Posix has offered free parking at http://vweb.co.za for over
a year (and its accessible via IPv6!)

I'll bet that many ISPs do pretty much the same thing.
