I'm in the same boat (far-north suburbs, actually (too far for DSL)).  A
former cow-orker suggested taking a look at att.com.  I just requested a
quote today and am supposed to have it by Monday (the quote).  Can't say
anything re: reliability.  I'll let you know how the quote comes out.  If
interested, I spoke with Vince Simone, (866) 932-4932x1393.  BTW, he
mentioned something about 3 months free, if I heard him correctly, and free
installation.  Comes w/ a Cisco 2600-series router.  If you contact him, let
him know I sent you.

I also got a quote from SBC/Ameriwreck (ameritech.com).  w/ 3
yr contract, Cisco router (model unknown), managed/monitored by them, it's
$400 setup, $875/month.  Unmanaged/no equipment, $400/$800.  For a 1 yr
$contract, the price jumps to 400/$1269 and $2715/$1228 (no, that's not a
$typo - one of these things is not like the other).
