I also got a quote from SBC/Ameriwreck (ameritech.com). w/ 3 yr contract, Cisco router (model unknown), managed/monitored by them, it's $400 setup, $875/month. Unmanaged/no equipment, $400/$800. For a 1 yr $contract, the price jumps to 400/$1269 and $2715/$1228 (no, that's not a $typo - one of these things is not like the other).
