It turns out that if getmail is passing along the emails to an MTA and the MTA --in my case procmail-- writes to std err then getmail will not delete the messages. I had an innocuous error in my procmail recipe that was writing to std err.  Once I corrected it, getmail began deleting the messages after they were downloaded.
