  Not that I've done any of this, except under windows one-on-one, but a 
question comes to mind.

  Shall we assume a one-to-many relationship?  You want to show the users 
your screen, but not theirs?  or do you want to let them control your screen 
or share their screen?  The fact that you mention streaming video suggests 
the one-to-many.  The desktop sharing usually implies a bi-directional flow. 
I'm sure these can both be done at the same time, but I'm not sure of the 
value or your specific situation.  Maybe we could think of it as a desktop 
broadcast.  This might be possible with VNC...

  For the voice, you could consider a VoIP conference call (or a regular 
conference call).  Asterisk suggests itself to me for VoIP conferencing, but 
it is hiting the nail with an oversized hammer.  You have bandwidth vs phone 
line considerations.  Of course there are 3rd party solutions for both of 
the video portion and the voice portion of the need.

  If, by application, you mean software written, then I have to say, I'm not 
sure there is a need to write software, but if you mean the application from 
the point of view of applying certain software to solving a need, then 
design away.

  I'm looking back over this and it doesn't read quite politely, but I can't 
quite fix it.  I think the points are valid, though.  I messed up my foot 
this weekend and it is a bit painful.  Please forgive any perceived 
gruffness.  It is not intended.
