It's in use.  A place I worked for used it for internal video broadcasts,
and it worked pretty well.

A quick google gave me http://www.multicastvideo.com/ , "The Multicast
Video Channel page provides a list of permanent video multicast streams".

I don't have answers to your other questions, hopefully someone on the
list will.
