the problem you have is the upstream is only 128.

even with QoS, a 1500 byte packet (MTU 1500) will take around 40ms to
get through, which means any VoIP packet can suffer up to 40ms delay.

Drop the MTU on your router (and probably everywhere else you can) to
576. this will result in slightly more traffic due to the higher
overhead, but will reduce the delay.

XBox live will refuse to work with an MTU that low, I think everything
else is fine though.

I'm not sure if the D-Link will be acknowledging QoS packets either. I
have always found the best results for VoIP with Billion modems and
routers.
