The process for doing this is documented in the constitution[1] (section 33, "APPOINTMENT OF PROXIES") - but in short, there's a small form you need to fill out and send to both your proxy and the secretary prior to the AGM. In the interests of openness I'd suggest CCing the activities list as well.
