I don't think that we absolutely need to use the PDF generator for
every single page view of the PDF file, and I can't imagine that we
need to take great pains to keep every single PDF for every single
book completely up-to-date. It may be good enough to simply create a
new PDF when one is specifically needed, and then we do't need to
worry about the drain on the toolserver.
