This has taken a while to track down, but it appears to be buggy firmware
and/or controllers in some batches of Samsung NVMe drives, disconnecting
completely from the bus:

- Samsung SSD 990 PRO 2TB 0B2QJXG7
- Samsung SSD 990 PRO 4TB 0B2QJXG7

They're not disconnecting due to excessive heat; Samsung advises applying

2025 update is supposed to help address this.
