Log:
  Formatting and style pass through the 2016Q2 report
  
  Capitalize FreeBSD in FreeBSD.org domain names and email addresses,
  and use &os; most elsewhere (but not for the Foundation).
  Use title case for <url> bodies (and keep newlines out of them).
