pccard.conf entry for SMC ethernet card

I picked up an SMC "EZ Card 10/100" 'cause it was cheap and I was tired of dealing with the dongle on my trusty old Linksys combo card, but I can't remember how to build a pccard.conf entry from the dumpcis values. Can someone remind me?
