Don't know what you have already tried, but my suggestion is:
device pcm0 at isa? port 0x220 irq 5 drq 1 flags 0x10

Do not include device csa or options PNPBIOS.

Then cd /dev; ./MAKEDEV snd0

This is only a guess as I have a different laptop with the same sound
chip.
