Looks like this card is a TI ACX based part.  Both from the driver
disk, which has things like tiacxgen.bin, tiacxmx.bin, tiaxcrmx.bin
and tiacxsn.bin on it, as well as the vendor/product number of 0x104c
and 0x8400.  0x104c is TI.

Just FYI.  Another $75 down the drain :-(.
