I use a:

"3COM Megahertz 56k Global GSM & Cellular Modem PC Card with XJACK Connector"

Model #: 3CXM756

You can get these for less than $30 shipped to your door
from pricewatch.

I have only tested the 56k functionality, not GSM, fax,
or voice.

56k works great. Modem dialing sound even comes out of my
laptop's speakers! I've tested it with 5.1-RELEASE and
5.2.1-RELEASE.

Here's what it looks like in dmesg:

sio4: <3Com Megahertz 3CXM756/3CCM756> at port 0x3e8-0x3ef irq 9 function 0 config 34 on pccard0
sio4: type 16550A
sio4: unable to activate interrupt in fast mode - using normal mode

I think it's a bit of a power hog though. Don't dial up
unless you're on AC.
