www/angie-module-jwt: update 3.4.4 => 3.4.5

This patch updates the port to the latest version 3.4.5

Build/install/run tested OK on 14.4-RELEASE

The patch assumes the state of angie* ports with the patch from bug #297154
already applied (update www/angie 1.12.0 => 1.12.1)
