nvidia-drm: fix freebsd page offset during nvkms/user memory faults

I'm not 100% it could fix this issue but I do have the following fix I've been
running with for this part of the code. Patch file attached, you'll have to
either patch the NVIDIA tarball or pull it into your port build.

I did look into a similar problem a while ago when testing kwin_wayland and
merged a fix, so it should have been working at some point. I haven't tested it
in a while.
