Hard drive failure?

While booting my system two days ago, I got some looping errors and boot failed. After that, I could no longer boot the OS.

I did a manufacturer memory test that came out clean.

To test the disk, I wipe the system and installed Linux Mint, the installation went fine and disktest reported the following, https://imgur.com/xyjzhKD

Again, I cleaned up the disk and tried to install NetBSD but something is wrong :(

The messages are the same as when it first failed.

Any ideas?

Is the disk failing or something even more serious?
