Please review wip/lapack340

Hello,

math/lapack is outdated, missing dozens of bug fixes and new functions.
I have used math/lapack as a template for the current lapack-3.4.0.
Fortran support in pkgsrc needs some fixes and updates, so this only
works with lang/gcc46 right now. Please take a look at the TODO file
for details. There are several ways to implement this package, as it
has so many options and different ways to be used. I import it in the
hope that it encourages other users of LAPACK to think about the best
way to update math/lapack.
