changes: clean up

Fix CSS in changes/2022 by including the page in layout.xml.

Use a consistent title format for the yearly changes pages.

Remove the cross references from the yearly changes pages, as the full
cross references were only maintained until 2012, creating the wrong
impression that NetBSD development has stopped back then.  The pages are
still reachable from the changes/index page.

In the yearly changes pages starting with 2011, replace the recent
pkgsrc changes with a direct link to the CHANGES-yyyy file on the CDN
server, which is more appropriate than linking to the now-current
changes.
