Do you have the reference manuals? They are still available in PDF format 
from NXP, although usually easier to find by just googling e.g. "68030UM 
pdf" than on their website's search thingy. At least they have an entire 
chapter covering the mmu and mmu instructions, although I don't recall if 
there are any concrete examples of setup sequence.
