I saw this on a 5000/240 I've been running more-or-less nonstop since
1999, and I never quite narrowed down what it was, but it seemed to be
related to some subtle corruption of a self-compiled libc.  I remember I
could shift symlinks around in /usr/lib and make the problem dissapear
and reappear at will in a couple dynamically linked apps.

I never did figure out exactly what it was.
