Thanks, that's all helpful information.

With the advice from you and John Nemeth I've solved the problem: a bad
serial cable (transmit but no receive at the Ultra 60 end) had fooled me
into trying connecting the keyboard.  Replacing the serial cable and
removing the keyboard got me into the installer.
