To answer one question before being asked: No, there's no design document 
regarding the DDM and the disk device API. The design was mainly discussed in 
June and July 2003 (!), the previous API version even earlier. Here are the

There's an overview draft mail initiating the final part of the discussion 
for the current API version:

It's certainly not fully up to date anymore, but it should still be close 
enough to the current implementation that it's worth reading. Scanning the 
follow-up mails for changes or new design decisions certainly doesn't harm 
either. :-)
