In an ideal world you would connect the shilding only at one side and
use a separate line for ground connection, to avoid current on the
shilding... Ghosting is more a problem of no shilding at all or
missmatch of cable impedance.

Yes, there are monitor ID pins. I found that it is no problem when they
are open. Halfway newish equipment doesn't use them anyway in favor of
DDC. But if DDC is missing it doesn't hurt either.
