Thanks to all for the suggestions. My best guess is that the original root
slice was corrupted in a block that affected the kernel load. I would have
expected the patch cluster to fail but that wasn't the case.  I was unable to
boot off of the upgraded disk or the non-upgraded disk. After booting from CD
I fscked the root slice on both drives and it found a lot of issues.

My fix was to steal a drive from the twin, which was already upgraded, to do a
recovery.

All,

I have a  V240 running 5.9 it was at patchset 122300-52 and I applied  the
latest 64 bit 9_Recommended patchset (122300-61).  My process was to detach
one side of the mirrored drives and apply the patch. Reboot the host and when
all is well, resync the mirrors.

All of the patches that needed to apply did, meaning I didn't see any abnormal
errors on the console or the log. However, when I reboot the host I get the
following error:

I get the same message (Fast Data Access) booting from disk1.  So far I have:

1)      Setenv auto-boot? False ; reset-all

2)      Complete power off and restart (POST tests all PASSED)

3)      Reinstalled the boot-block on both disks.

4)      Can boot from cdrom (which I think rules out hardware).

I applied this same patchset to an exact twin (Hardware and Software) last
month without issue. I don't know where to go from here, any help is greatly
appreciated.
