Installing 4.0E on XP1000 from CD

As it goes thru the boot get following error:

cam_logger: CAM_ERROR packet
cam_logger: bus 1 target 1 lun 0
ss_perform_timeout
timeout on request on the bus, scheduled bus reset
Active CCB at time of error

and it tries several times before trying to proceed
with boot.

It does get to the following but the errors continue

vm_swap_init: warning /sbin/swapdefault swap device not found
vm_swap_init: swap is set to lazy (over commitment) mode

I have swapped drives reseated cables but no change
