QB-5TWAB-SA kit really required for RA3000?

Dear managers,

I'm trying to revive a RA3000 attached to my DS20 under 4.0F PK8. So far,
I can see the 4 redundany groups I configured via the serial interface
on the RA3000 on the SRM and the scu. The device nodes are in place,
created by doconfig -d.

The problem now is that access to the device(s) via disklabel, dd,
"scu format" fails (see also at the end of this message), e.g.

bash-2.05b# scu -f /dev/rrz16c
scu> format
scu: 'mode sense (6)' failed, EIO (5) - I/O error
scu: Sense Key = 4 = HARDWARE ERROR - Nonrecoverable hardware error,
Sense Code/Qualifier = (0x40, 0xff)
Formatting device /dev/rrz16c (HSZ22), please be patient...
        Working ... Mon Nov 22 20:36:17 2010
scu: 'format unit' failed, EIO (5) - I/O error
scu: Sense Key = 4 = HARDWARE ERROR - Nonrecoverable hardware error,
Sense Code/Qualifier = (0x40, 0xff)
scu>

Did I miss something? What else do I need? Maybe (parts of) the
QB-5TWAB-SA "RA3000 Solution Kit" ? If yes, can somebody provide it to
me, or least the software required for Tru64?

Many thanks
Urs
