Capacitor failures on DC input to FT100

Damaged capacitors C3074 and C3076 were found on the underside of he PA board while troubleshooting.  These capacitors failed with resulting fire damage to the PC board under the capacitors.  The capacitors are rf bypass capacitors directly across the 12 volt supply feed to the FT100 and are always connected.  C3074 is a chip capacitor 0.1 uf rated at 25 VDC.  C3076 which is adjacent to C3074 is a chip capacitor 0.001 rated at 50 VDC.  I suspect that a transient on the 12 Volt line exceeded the 25 Volt rating of C3074 and the resulting heat and arc destroyed C3076.  Smoke and arcing damage are present in the area.
     I'm in the process of repairing the PC board traces and have ordered new capacitors with higher voltage ratings.  The use of a capacitor with a 25 VDC rating directly across the 12 DC input to the FT100 is not good design.  In automotive circuits, the accepted practice is to rate everything attached to the 12 VDC feed at 70 VDC or higher to handle transients.
     The capacitors which I am replacing these with are as follows:
 C 3074  0.1uf  -- new rating 100 VDC  X7R 1206 20% tol Mfg AVX 12061C104M4T2A
 C 3075  0.01uf  -- new rating 100 VDC  X7R 1206 20% tol  Mfg AVX 12061C103MAZ2A
 C 3076  0.001uf  --new rating 100 VDC  X7R  1206 20% tol Mfg  AVX  12061C102K4T2A
 These were in stock an Mouser at a price of about $0.25 each ant a quantity of 10 each which is a very low price to prevent further possible damage to the FT100.  

 I still have a bit of cleanup and trace repair due, but I'm looking forward to getting this back on the air.
