The parent element is for example <chapter>,
but if <indexterm> is an inline element, then I only
must avoid this tag between two <para> tags.
For example by putting the <indexterm> in one <para>
tag.

<para>
    <indexterm><primary>ice cream</primary></indexterm>
    Let us have some ice cream. ...
</para>

Then every thing works fine.
