Pure-XSLT implementation, take 2

Well, guess what.
I've got basic DocBook->man output with XSLT alone,
and so far the output looks perfect, no spurious blank lines anywhere :)

(It really was a good design decision to use the Man-XML intermediate format;
this helped immensely in debugging.)

No tables though, unless someone screams for it -- the table algorithm is
complicated enough; I'm not in the mood to reimplement it.

The code is in CVS, look under the directory xslt/backend.

Things left to do for version 0.8.7:

 * better documentation of stylesheet params
 * write the performance analysis section, as I described in previous
message to this list
 * testing! (i.e. verify round-trip conversion of thousands of man pages)
